To complete the final step so QCPS may access your NMLS Dashboard portal, please click the  online login information form button  or click the download login information form button  below to print. Please enter your login information and contact information on either of these form. Sign it and send it to us via online or fax. Once the MCR desk receives your information, you will be notified via email.

NOTE: We will only download the years and quarters you requested on the form. We will login to your NMLS account and download your filed MCR reports, complete your test, and email our results to you within 72 hours or sooner.
